Neeraj Sharma, MD, FACP, MBA
Founder and CEO

Dr. Sharma brings over 25 years of clinical, organizational and executive experience in healthcare, from acute hospital to post-acute and transitional care settings.
Board certified in Internal Medicine as well as Hospice & Palliative Medicine, Dr. Sharma has served in leadership positions for and consulted with hospitals, home health and hospice agencies, and post-acute care facilities.

Dr. Sharma founded Curate to bring together experience, effort and innovation in the post-acute and long-term care space. Dr. Sharma graduated from Duke University and earned his medical degree and training in the University of Texas health system, followed by an MBA in Healthcare Management from the University of Texas at Dallas. He earned fellowship in the American College of Physicians and the American Board of Quality Assurance and Utilization Review Physicians, and holds the Certified Medical Director status from the Society for Post-Acute and Long-Term Care/AMDA.
Padma Soundarraj, APRN-CNP
Nurse Practitioner

Ms. Soundarraj grew up in Chennai, India, earning a degree in Microbiology before emigrating to the US. She started as a nursing assistant with a geriatric focus, before earning her Bachelor of Science in Nursing from Illinois State University in 2013. Ms. Soundarraj brings extensive experience in various healthcare settings, including Cardiology clinics, post-acute and long-term care units, and rehabilitation centers.

Ms. Soundarraj contributes years of experience in infection control, and as an Assistant Director of Nursing at a post-acute care facility. She graduated with a Master's Degree in Family Nurse Practice in 2019 from University of Texas at Arlington, and joined our team.
Ms. Soundarraj believe in not just treating patients but helping patients and family members to make informed decisions. When not working, she enjoys gardening and cooking. She lives in Frisco with her husband of 20 years, two boys in college and a dog.
Vanessa Sylvia, APRN-CNP
Nurse Practitioner

Ms. Sylvia has extensive experience in a variety of healthcare settings, including hospital intensive care units, emergency departments and post-acute care.
​
Ms. Sylvia brings a comprehensive nursing educational background, includes a Master's Degree for a Family Nurse Practitioner, certification from the American Association of Nurse Practitioners, and prior certification as a wound care specialist. Her NP career follows 16 years' experience in hospital nursing as an LVN then RN.
Ms. Sylvia is a proud "Marine Mom" and loves spending "Nana" time with her young grandchildren.
Shandalyn Eastham, APRN-CNP
Nurse Practitioner

Ms. Eastham has extensive experience in a variety of healthcare settings, including hospital intensive care units, emergency departments and post-acute care.
​
Ms. Eastham started her career at age 18 as a certified nurse aide in a long-term care facility, before pursuing her further education first as an LVN and then earning her RN, continuing on the earn her Master's Degree in Nursing and eventually her credentials as a Family Nurse Practitioner, before joining our practice. She believes that while each work experience has shaped her practice as an Nurse Practitioner, she was glad to be back to her true passion of working in the post-acute and long-term care setting.
Ms. Cerda enjoys much of her free time as a "softball mom".
Quinta Fonkwa, AGPCNP-BC
Nurse Practitioner

Ms. Fonkwa has extensive experience in a variety of healthcare settings, including hospital and post-acute care rehabilitation units in Ohio and now Texas, Oncology units, and quality management programs administered by large health systems for the benefit of long-term care patients.
​
Ms. Fonkwa brings years of experience in Nursing and nursing management, graduating in 2016 with a Bachelor's Degree in Nursing from the University of Texas at Arlington, before going on to complete her Nurse Practitioner training in 2022. She is the latest addition to our team.
