Our Team of healthcare professionals provides the highest level of evidence-based, effective, efficient and compassionate care to patients in the post-acute care (upon discharge from a hospital), long-term care (for residents who need a nursing facility to care for them indefinitely), and residential care (for residents in senior living facilities including group residential care homes, assisted and independent living, and memory care).
